Notice of Privacy Practices Effective Date: February 16, 2026

Summary of Your Rights: This notice describes how medical information about you may be used and disclosed. As of February 2026, new protections apply to your data.

Special Protections for Substance Use Disorder (SUD) Records: In accordance with 42 CFR Part 2 and the 2024/2026 Final Rules, if we receive or maintain SUD treatment records, they are subject to stricter protections than standard medical records. These records will not be used or disclosed in civil, criminal, administrative, or legislative proceedings against you without your specific written consent or a specialized court order.

Your Rights:

  • You have the right to request a restriction on certain uses and disclosures of your PHI.

  • You have the right to opt-out of any fundraising communications.

  • Notice of Redisclosure: Once information is disclosed pursuant to your authorization, it may be subject to redisclosure by the recipient and may no longer be protected by federal privacy rules.
